Lipedema

Chronic disease of subcutaneous adipose tissue called lipedema is often mistaken for obesity, and thus not only by the general public, but unfortunately still also by some doctors. The aestea clinic has been a pioneer in the treatment of lipedema for many years, thanks to which its help is sought by clients not only from Europe, but from all over the world. Not familiar with the diagnosis of lipedema? Maybe when reading the description of the symptoms, you will find yourself or someone you know!

Diagnosis of lipedema

Lipedema is most commonly found in the lower parts of the body, specifically the buttocks and lower legs, as well as the arms. However, it can also occur anywhere else on the body. On the lower limbs, a sharp termination can typically be seen at the knees or ankles.

The limbs affected by lipedema are symmetrically enlarged, sometimes even deformed. In lipedema, there is impaired metabolism, stagnation of interstitial fluid and oppression of lymphatic and blood vessels, leading to swelling. This accumulated fluid also causes tenderness and pain by pressing on nerve endings.

Lipedema almost exclusively affects women, because the fat cell overgrowth is related to levels of the female hormone estrogen. Its occurrence is genetically determined.

Conventional reduction diets and exercise have no effect on the reduction of lipedema tissue.

Lipedema was not recognised by doctors and especially health insurance companies for a long time, and even today it is still not widely known.

The symptoms of this disease are fat multiplication in the thighs, knees, shins, but often also on the arms, forearms and other places. These visible signs are accompanied by pain in the affected areas and easy bruising.

Despite ongoing research, the cause of the disease is still not clear. There is a clear genetic disposition and disease progression is common.

Usually 3 stages of the disease are described, from stage I – thickened subcutaneous tissue through stage II – uneven surface to stage III – fatty bumps and bulges deforming the shape of the limb.

The recommended conservative procedures – lymphatic drainage, wearing compression underwear – are an essential part of holistic and long-term therapy, however, in terms of scope, speed and effectiveness they do not represent such a significant and long-term modification of the condition. According to our experience, only liposuction, often liposuction of large volume, can bring the most significant benefit, the most visible effect and immediate improvement in quality of life.

A holistic approach

The treatment and prevention of lipedema is a long-term, often lifelong process and requires a holistic approach, i.e. an approach involving multiple areas of the patient’s life, with the necessary assistance of a range of medical and non-medical professions. In justified cases, liposuction is recommended as an adjunct to conservative procedures.

The aestea clinic specializes in high-volume liposuction for the treatment of lipedema. According to our experience, it not only corrects the aesthetic deformity of the body by significantly reducing fatty tissue, but also reduces pain, allows active movement and generally improves the quality of life of patients with lipedema.

Even in other therapeutic areas, aesthea tries to assist its clients by providing advice and information at a minimum.

An important part of a holistic approach to fighting lipedema is proper nutrition. This is specific to lipedema patients and different from “classic” overweight patients. Although it may seem paradoxical to many, the most common nutritional problem of lipedema patients is their actual malnutrition, when their diet does not provide sufficient nutrients, especially protein. It has been shown that this problem is often overlooked in routine clinical examinations.

Aestea therefore recommends a nutritional consultation with a specialist who is oriented in the field of lipedema and chronic swelling. This specialist will set up and then regulate and coordinate the correct nutritional treatment in the long term. We recommend our clients to contact with confidence the Centre for Preventive Medicine in Prague, led by René Vlasák, MD www.prevence2000.cz.

The fact that movement is an integral part of any therapy related to body weight and adipose tissue is a matter of common knowledge. However, in the context of lipedema, a number of specifics must also be taken into account with regard to movement. Movement therapy for patients with lipedema must be aimed at activating the muscles used to improve venous return of blood to the heart and accelerate the flow of lymph from the areas affected by lipedema. To achieve this so-called lymphatic drainage, a set of exercises known as drainage exercises is used. The therapy includes breathing exercises, exercises of the pelvic floor, buttock and hip muscles up to the calf muscles, which are considered the heart of the lymphatic system. It is advisable that an individual exercise plan be developed for each patient by an experienced physiotherapist familiar with both the issue of lymphatic insufficiency and the issue of hypermobility, which is often found in patients with lipedema.

Compression is another of the mainstays of lipedema treatment. We distinguish between its application immediately after liposuction and subsequent compression, which is already of a preventive nature. The application of postoperative compression garments is an essential part of every procedure/liposuction. Patients leave the aestea clinic after each liposuction procedure equipped with suitable underwear, which the clinic staff selects and tries out with them. Also for long-term compression treatment of lipedema, so-called compression leggings are applied. These have a positive effect on the microcirculation in the subcutaneous connective tissue, speeding up lymphatic drainage, which leads to a reduction in swelling, flushing out the waste products of cell metabolism, and this has a positive impact on the areas affected by lipedema. Also, compression leggings, which are recommended to be worn for long periods of time, at least an hour a day, can be tried on and ordered at the aestea clinic.

This special treatment method improves or speeds up the flow of lymph through the lymphatic system and then into the venous system. The lymph with the lymphatic (lymphatic) system represents the internal ecosystem of the body. The functioning of this system has a major impact on the quality of the environment of the cells and tissues and thus on the health of the whole organism. In the treatment of lipedema, it is recommended to combine both methods of lymphatic drainage, i.e. manual and instrumental. Lymphatic self-treatment is considered important, which is a set of simple exercises and self-massage movements that are likened to personal hygiene habits. Lymphatic drainage should always be discussed with your chosen specialist. We recommend that our clients contact one of our certified lymphatic centers https://www.lympho.cz/lymfocentra.

An effective part of drug therapy is also the application of special gels directly to the areas affected by lipedema, both in connection with liposuction, as well as part of long-term/life-long therapy.
The aestea clinic has teamed up in this area with the top laboratory BIOCEN Laboratories s.r.o., which has developed the AESTEA LIPO GEL exclusively for us. This product is intended for application not only in the period immediately after liposuction, but also on a long-term basis. AESTEA LIPO GEL, as its name suggests, has a complex positive effect and benefit for the patient, improves the course of healing, reduces pain, swelling and other negative effects associated with the procedure and contributes to the maximum positive effect of the procedure.

Drug therapy focuses on microcirculation and promoting venous return and lymphatic drainage. The current trend in the treatment of lipedema is the so-called personalized form of administration of phytopharmaceuticals, both topical and oral. The purpose of this complex treatment is to positively influence the homeostasis (stability of the internal environment) of the whole organism.
Pharmacological treatment should always be consulted with your attending physician as well as a specialist in the treatment of lipedema. We recommend our clients to contact the Centre for Preventive Medicine in Prague led by René Vlasák, MD www.prevence2000.cz.

What helps

Liposuction

According to our experience, significant reduction of adipose tissue by liposuction can not only correct the aesthetic deformity of the body, but especially reduce pain, allow active movement and generally improve the quality of life of patients with lipedema.

How the examination is carried out

In the case of lipedema, we cooperate with specialists from other disciplines to establish the diagnosis. We can usually recommend surgery on the basis of clear photographs sent to us, together with other information, especially height and weight data, but also information about your health. The final decision on the extent and location will of course only be made in a personal consultation with your surgeon prior to surgery.

What is the preparation before the procedure

In preparation for surgery, it is necessary to:

  • discontinue all medications causing increased bleeding
  • to reduce the risk of embolism, consider discontinuing contraception three weeks before the procedure
  • maintain a 2-3 week interval since the last infectious disease
  • in order to improve healing before and after the procedure, do not smoke
  • 6 hours before the procedure, do not drink or eat anything
  • not to underestimate cleansing and hygiene on the day of surgery, not to wear make-up
Procedure of the procedure

The basic condition is a sufficiently large and highly trained, experienced operational team. Two to three surgeons operate simultaneously. General anaesthesia is administered by an experienced anaesthesiologist, who carefully monitors the patient’s condition and his reaction to the procedure, pays for fluid losses and ensures that the patient does not catch a cold.

aestea uses the so-called super-wet-technique, in which the same fluid is used for infiltration as in the tumescent technique, only in smaller quantities. The amount of fluid infiltrated is approximately the same as the amount of fat suctioned during the operation.

Small incisions of about 4 – 5 mm in length are made in suitable and later difficult to recognise places to allow the cannulas to enter. The cannula is first used to inject fluid into the tissue and then, with regular movements, the released fat is suctioned under vacuum into special containers. The operated areas are compressed with compression linen at the end of the procedure.

What is the postoperative care

Anti-nausea drugs are administered as a precaution during the operation, pain killers are started immediately after the operation, unpleasant cold sensation is overcome by intensive warming with special hot air heating and warm pillows.

What is the postoperative care

Large volume liposuction is a sensitive and extensive procedure and the post-operative follow-up and care corresponds to this. We strive for maximum comfort of our clients. Experienced nurses monitor the overall condition and are supportive in managing the discomfort of the post-operative period.

The operation is usually followed by a one-day stay in the clinic.
Pain, which is relieved by analgesics, and nausea should be expected.
In the first two weeks after the procedure, it is necessary to limit physical exertion.

Each client leaves our clinic fully informed and familiar with all possible and expected symptoms of the postoperative condition. If in doubt, the client is free to contact the surgeon at any time.

We recommend taking a 1-2 week holiday. You can return to work after 7-14 days and to more or less normal life after 2-3 weeks.

Results

Before and after the procedure

Photographs of the operated areas taken before and after the procedure perfectly illustrate its optical effect. However, for many procedures, we should not forget about their health effects, such as increased mobility, pain reduction, etc.
